              Company Safety Profile Help  Using the Company Safety Profile (CSP) Request form, you can request a
		   report that contains a summarized account of all inspections and 
		   accident reports involving the requested motor company.
		   For a more detailed description of the CSP and the information it 
		   contains, refer to the full CSP documentation.
		   To successfully obtain a Company Safety Profile, please review 
		   the following information.
 
 
 
			Enter USDOT # and E-mail AddressEnter the USDOT # of the company of interest and the email 
			    address where the Profile should be returned to the requestor
				in the appropriate text boxes on the Company Safety Profile
				Request Form.
 
Select Whether You Want Driver Information Or NotIf driver information is to be included in the requested 
			    Company Safety Profile, select the "I am the company..." 
				checkbox on the Company Safety Profile Request Form. Due to 
				FOIA, driver information will only be released to the motor 
				company itself or one of it's duly appointed agents. This is 
				enforced by ensuring that the e-mail address, supplied in the 
				appropriate text box, matches the authorized e-mail address(s)
				of the company whose USDOT # was supplied in the box and which
				is stored on the MCMIS system. To further ensure 
				confidentiality, the requestor will also be asked to provide
				the last 4 digits of the company's Tax Payer ID number.
 
Click SubmitAfter supplying the USDOT # and e-mail address, click on the
			    submit button.  If driver information was requested, the
				requestor will be prompted for the Tax Payer ID number 
				mentioned above.  As a final check, the requestor will be 
				presented with the name and address of the company associated
				with the supplied USDOT #.  If the information is correct and
				what you expected, press the submit button.
 
Credit Card PaymentYou will then be directed to a secure site for entering credit
			    card payment information.  After entering this information,
				the secure server will approve/decline your payment and 
				automatically forward you back to a confirmation page that will
				inform you whether or not your request was successfully
				submitted.
 NOTE: After the approval/decline, your browser may 
				give a warning about being directed to an unsecured site.
				This is normal and should be responded to with an "OK".  All
				personal information was transmitted in a secure mode.  The 
				warning is just letting you know you are being returned to the
				original, un-secure site.
 
 
Receive Notification via E-mailAssuming the e-mail address provided on the Company Safety 
			    Profile Request Form was your e-mail address, you should 
				expect to receive within 72 hours an e-mail message that will
			    provide you instructions on how to download your company
				Safety Profile.
 Please Note:
 
		    Due to the nature of the servers involved, profile requests may
			    take up to 72 hours to be fulfilled.
Any Company Safety Profile questions should be directed to the
			    FMCSA Information Line at 1-800-832-5660.
Enforcement users should use the CSP Request form located on 
			    the Enforcement page by selecting "Enforcement" from the 
				"Business Services" menu.
